TORONTO, November 15, 2023 – PocketHealth, the leading patient-centric image exchange, today announced the launch of MyCare Navigator, an advanced tool within PocketHealth to give patients a deeper understanding of their healthcare and next steps to keep that journey on-track. 

MyCare Navigator strengthens patient-provider connections by encouraging follow-up adherence, improving communication, and offering health screening tools to keep patients engaged and actively involved. PocketHealth streamlines healthcare operations by saving staff time, lowering operational costs and fostering lasting patient relationships.

MyCare Navigator Features

  • Ask My Doctor gives patients personalized questions to discuss with their physicians, tailored to the content in their medical reports. Ask My Doctor equips patients with the confidence and tools to engage in critical conversations about their health, while making the most of the limited time they have available with their healthcare provider.

  • Follow-Up Navigator is a powerful feature that identifies and extracts any follow-up recommendations that are contained within the report and surfacing them to the patient. When a follow-up recommendation is indicated in their report, patients will also be notified within MyCare Navigator, making patients promptly aware of important next steps to manage their health. 
  • Bone Health Screening, offers eligible patients a proactive assessment of their clinical risk for osteoporosis. Using a clinically validated tool known as Rho, it provides users with a personalized score, ranging from 1 to 10, that indicates their risk of low bone mineral density. Additionally, patients receive a comprehensive report with lifestyle and treatment recommendations to maintain optimal bone health.
